Uploading Documents to Patient Charts

Add supporting documents to enhance AI accuracy and create more comprehensive progress notes

Before You Begin

Important: You must link a patient to your session before uploading documents or starting a recording. If you don't link a patient first, uploaded documents will not appear or be accessible.

When to Upload Documents

You can upload documents to a patient's chart in two ways:

Option 1: Documents Section

  • Navigate to the patient's chart

  • Go to the Documents section

  • Upload files directly to the patient's record

Option 2: During Recording Sessions

  • Link your patient first

  • Upload documents while using the Recorder

  • Documents must be uploaded before starting the recording

Using Previously Uploaded Documents

If you've uploaded documents to a patient's chart before, you can easily access them when:

  • Writing a note

  • Uploading audio files

  • Starting a new recording session

Simply select the relevant documents from the patient's existing files (patient must be linked to access these files).

How Documents Improve Your Notes

Uploading relevant documents significantly enhances the AI's ability to create accurate, detailed progress notes. Consider uploading:

  • Lab work and test results

  • Intake forms

  • Previous medical records

  • Imaging reports

  • Specialist consultations

The AI uses this additional context to produce more comprehensive and clinically relevant documentation.

File Requirements and Credits

Supported Files:

  • PDF files only

  • Maximum file size: 25MB

Credit Usage:

  • Each uploaded document uses 0.25 additional note credits

  • Documents over 10 pages use an additional 0.25 credits for each extra page

  • Credit terms may vary if your clinic has a specific agreement with us
